Job summary

Greenfield Rehabilitation Agency, Inc. is seeking a licensed Physical Therapist to join our home care team in Wauwatosa, WI. Full-time or part-time options available. Provide comprehensive evaluations and develop individualized treatment plans for seniors in the home setting.

You will deliver evidence-based therapy, supervise PTAs as appropriate, and collaborate with an interdisciplinary team to achieve exceptional outcomes. Strong documentation and communication skills are essential.

Qualifications

• Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
• Certification or license as required by the State of Wisconsin to practice as a Physical Therapist.
• One year practicing as a Physical Therapist.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans including OASIS paperwork.
• Deliver high-quality, evidence-based physical therapy services.
• Supervise PTAs as appropriate.
• Collaborate with an interdisciplinary team focused on exceptional outcomes.
• Build strong relationships with clients and families.
• Maintain accurate and compliant documentation.
