Greenfield Rehabilitation Agency is looking for a passionate PRN Physical Therapist to join our team supporting senior communities in Pigeon Falls and surrounding areas. As a PRN PT, you’ll provide skilled, patient‑centered care across skilled nursing facilities, assisted living communities, and in‑home care, helping residents improve mobility, strength, and independence while enhancing quality of life.

Your Role

• Conduct initial and ongoing evaluations to assess mobility, balance, strength, and function.
• Develop and implement individualized treatment plans.
• Provide hands‑on therapy to help residents recover, maintain independence, and prevent falls.
• Educate residents, families, and staff on safe mobility and functional activities.
• Collaborate with PTs, OT, SLPs, nurses, and other staff to ensure coordinated, holistic care.
• Maintain accurate documentation of therapy sessions and outcomes.
