Upstate Caring Partners, Inc. is seeking a Registered Physical Therapist to join our Central New York team. You will provide expert evaluation, therapy services, and collaborative care across our programs to help individuals improve mobility and independence.

Must hold a NYS PT license, a Bachelor's in Physical Therapy, and have 1–3 years of experience. A valid NYS Driver's License and ability to travel between sites required. We offer a competitive benefits package and sign-on bonus eligibility.

• Maintain accurate documentation per program and regulatory requirements.
• Support departmental operations and orientation of new staff.
• Participate in agency-wide initiatives to advance mission.

Core Responsibilities

As a Registered Physical Therapist, you will support individuals across our programs by providing expert evaluation, therapy services, and collaborative care.

• Evaluate and assess individuals on an ongoing basis to develop personalized, meaningful treatment goals.
• Implement therapeutic interventions based on assessment findings, helping individuals enhance mobility, function, and quality of life.
• Educate individuals, families, and team members on treatment plans, therapy methods, injury prevention, positioning, range of motion, and more.
• Collaborate closely with interdisciplinary teams including educators and healthcare professionals ensuring alignment with each Individual Program Plan (IPP) or Individual Education Plan (IEP).
• Maintain accurate and timely documentation in accordance with program and regulatory requirements.
• Support departmental operations, including equipment needs, adaptation, and orientation of new staff.
• Participate in agency-wide initiatives that advance our mission and enhance the well-being of the people we serve.
