Job summary

This role offers 30 hours per week at an 88-bed Skilled Nursing and Rehabilitation Facility, with potential for a split schedule across two facilities and travel between locations if chosen. New graduates are welcome to apply and will be supported as they begin their clinical careers.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed or eligible for licensure as a Physical Therapist in Georgia.
• Strong clinical and communication skills.
• Patient-focused, dependable, and collaborative.
• Willingness to travel between facilities if pursuing the split/full-time opportunity.
• New graduates are welcome to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care
• Deliver skilled physical therapy interventions based on patient needs and goals
• Provide therapeutic exercise, mobility training, balance activities, functional training, and other appropriate treatments
• Monitor patient progress and adjust treatment plans when clinically appropriate
• Educate patients and caregivers on mobility, exercises, safety, and continued progress
• Maintain accurate and timely clinical documentation
• Collaborate with PTAs, nursing staff, and other members of the interdisciplinary team
• Communicate patient progress and relevant clinical findings effectively
• Provide compassionate, patient-centered care that promotes independence and quality of life
