Responsibilities

• Consult with patients to learn about their physical condition and medical history
• Diagnose movement dysfunction by observing patients and reviewing their reports
• Develop individualized treatment plans
• Coach and teach therapeutic exercises to relieve pain and improve mobility
• Supervise and support physical therapist assistants and rehabilitation aides
• Maintain accurate patient records and progress notes
• Utilize standardized assessments and evidence-based practice
• Provide discharge summaries and communicate progress to the care team
• Ensure regulatory compliance and complete documentation in the electronic system
