The incumbent will perform evaluations, design individualized treatment plans, lead therapeutic exercises, and monitor progress while maintaining thorough documentation. A valid PT license in the operating state is required.

Qualifications

• Active Physical Therapist license in the state we operate.
• License must be current and unrestricted.

Responsibilities

• Complete physical therapy evaluations and screenings
• Develop and implement individualized treatment plans to support recovery and mobility
• Provide therapeutic exercises and interventions to improve strength, coordination, and balance
• Monitor progress, document outcomes, and adjust care plans as needed
• Duties as assigned
