Job summary

Optimal Home Care, Inc. is seeking a licensed Physical Therapist to deliver in-home therapy across Colorado. This full-time position offers a $10,000 sign-on bonus and pay per visit, with flexible scheduling and a supportive team.

You will evaluate patients, develop plans of care, coordinate with physicians, instruct patients and families, and document progress. The company emphasizes compassionate care, ongoing education, and a values-driven culture.

Qualifications

• Holds an active license and can practice in the state of Colorado.
• Preferrable at least one year of experience as a Physical Therapist.
• Possesses adequate and effective written and verbal communication skills.
• Possess adequate and effective interpersonal skills.

Responsibilities

• Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
• Assists the Physician in evaluating client/patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
• Develops, documents, implements and re-evaluates a specific Plan of Care noting all treatment modalities and frequencies.
• Treats patients to relieve pain and/or develop or restore function to maintain maximum performance.
• Makes arrangements for out-patient services which cannot be provided in the home.
• Reports to the Physician the client/patient’s reaction to treatment and/or any changes in the client/patient’s condition.
• Records clinical/progress notes in the patient’s clinical record weekly. Instructs client/patients and their families in the use of prosthetic, orthotic and assistive devices (cane, walkers, wheelchairs, etc.).
• Instructs the family in the client/patient’s total physical therapy program and evaluates the home environment.
• Instructs other involved agency personnel in the care of client/patients receiving physical therapy.
• Attends appropriate case conferences and In-service presentations.
• Plans for patient’s discharge, prepares discharge summaries and instructions
• Participates in staff and agency development activities.
