Snoqualmie Valley Health in Snoqualmie, WA, is hiring a licensed physical therapist for full-time (and per diem) opportunities with a $5,000 sign-on bonus. You will plan, implement, and evaluate PT services aligned with the total care plan.

You’ll work one-on-one with patients, develop individualized therapy plans, assess ROM, gait, balance, and functional activity, and coordinate with the care team. A WA PT license and a Master’s or Bachelors in Physical Therapy are required.

Competenze

• Master’s or bachelor’s degree in physical therapy and WA license.
• One year of clinical PT experience.
• Excellent written, verbal and interpersonal communication.

Mansioni

• Obtain medical history related to current condition from patient and family.
• Evaluate ROM, muscle status, balance, gait, endurance and functional activities.
• Develop an individualized therapy plan and provide 1:1 PT sessions.
• Consult with physicians and other disciplines to coordinate care.
• Maintain timely documentation of patient progress and outcomes.
• Refer to other disciplines as appropriate and educate caregivers for home care.
