Job summary

Legacy Healthcare Services is seeking a dedicated Physical Therapist to join our not-for-profit senior care teams across multiple communities. You will provide personalized rehabilitative care, build strong patient relationships, and participate in care planning with a multidisciplinary team.

We welcome compassionate professionals with a PT license and a commitment to improving residents’ quality of life, with a Monday–Friday schedule and ample resources for professional growth.

Qualifications

• Degree in Physical Therapy required.
• Current Physical Therapy license in state of practice, or ability to obtain prior to start.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency and the ability to use EMR systems.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one with individualized private sessions.
• Ability to tailor treatment sessions to each patient.
