Rehabilitation Center of Sandalwood in Wheat Ridge, CO, is seeking a dedicated Physical Therapist for a full-time role in our in-house rehab program. The position offers a stable, team-oriented environment within a post-acute facility serving geriatric patients and long-term care needs.

Responsabilidades

• Screen and evaluate patients with physical functioning disorders; develop care plans.
• Provide rehabilitative, skilled, and medically necessary treatments.
• Perform rehabilitation procedures including manual techniques, gait training, therapeutic exercises, modalities, and wound care.
• Counsel patients, families, and caregivers regarding physical disorders.
• Prepare comprehensive discharge summaries consistent with regulatory and clinical requirements.

PHYSICAL THERAPIST - FULL TIME - REHABILITATION CENTER OF SANDALWOOD - WHEAT RIDGE, CO

The Rehabilitation Center of Sandalwood in Wheat Ridge, CO, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required.
