Pay or shift range: $90,000 USD to $105,000 USD. The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

What You’ll Need

• Bachelor’s or Master’s Degree required, Doctorate degree preferred
• Licensed Physical Therapist in the state practicing in
• Completed state regulated CEU requirement in appropriate state (NY, NJ)
• Experience/knowledge with an EMR software is preferred
• Excellent oral and written communication
• Strong clinical decision‑making skills
• Excellent work ethic and dedication to patient success
• Basic computer skills
• Works as a team member and individually with minimal supervision
• Makes a positive, professional demeanor at all times

What You’ll Do

• Collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies and third‑party payers, reflecting an understanding of reimbursement systems and their impact on patient care.
• This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team.
• Additionally record daily billing and follow department guidelines for productivity.
• Prepare for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses.
• Select and administer tests and measures.
• Synthesize evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
• Establish treatment plan based on evaluation results, goals and best practice. Collaborate with interdisciplinary team, patients and patient’s support system. Assess and re‑assess need for modifications to plan of care and goals.
• Identify need for consultations and/or refer the patient to another health care professional or health care services. Initiate and complete discharge plan.
• Provide education/training to patient and/or patient support system.
• Train on ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration.
• Responsible for treatment, plan of care, exercise prescription for patient caseload.
• Provide interventions to patients that are safe, effective, and in compliance with JAG.
• Musculoskeletal interventions – therapeutic exercise, joint mobilization and modalities.
• Provide data, note writing using Prompt EMR for all treatments provided.
• Oversee support staff of PTA, ATC, PT aides.
• Attend annual JAG Billing and Coding seminar.
• Promote JAG in the community through lectures/presentations.
