Job Description - Physical Therapist (250006PM)

Physical Therapist - ( 250006PM )

  • Organizes and conducts Physical Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities.
  • The Physical Therapist directs the patient's participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.
  • Performs evaluation(s) and develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum.
  • Educates patient/family/caregiver in applicable health information, treatment and prevention.
  • Initiates and participates in the discharge planning process.
  • Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
  • Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
  • Full time 40 hours per week.
  • 100% travel to the patient's homes.
  • Day shift.
  • The territory will be: City of Cleveland, Garfield, Shaker Hts., Euclid, Richmond Hts., Cleveland Hts., Mayfield Hts., Beachwood, Pepper Pike, Lyndhurst, University Hts., South Euclid, Bedford, Solon, Maple Hts., Twinsburg, Hudson, Chagrin Falls, Macedonia, Northfield and Pennisula.

Sign-on Bonus: This position is eligible for a $6,000 sign-on bonus to eligible candidates. Eligible candidates must be newly hired to UH and have at least 1yr of PT experience.

University Hospitals is offering a $6,000 sign on bonus, minus applicable taxes, for designated regular full-time PT positions to experienced PT's (1 years or more) hired externally.

New Grad PT Candidates

This position is eligible for a $4,000 sign-on bonus to eligible candidates. Eligible candidates must be newly hired to UH and have less than 1yr of PT experience.

University Hospitals is offering a $4,000 sign on bonus, minus applicable taxes, for designated regular full-time PT positions to new grad PTs (less than 1 year of experience) hired externally.

  • Graduate of an accredited Physical Therapy program required.
  • Evaluation and treatment skills necessary to provide Physical Therapy appropriate to the developmental and chronological age of patients on caseload.
  • Professional initiative, objectivity, good time management and interpersonal skills.
  • Physically, cognitively and perceptually able to provide patient treatment and able to assist in physical mobility of patients and equipment
  • Basic Life Support (BLS) required within 90 Days.
  • Licensed Physical Therapist, Ohio and/or Multi State Compact license required.
