Job summary

Granite Peak Home Health in Montana is seeking a licensed Physical Therapist to join our dedicated team in Butte. You will provide physician-prescribed therapy to homebound patients, develop plans of care, and collaborate with care teams to restore function. Requirements include graduation from an approved PT program, MT license, BLS/CPR, and knowledge of HIPAA and EHR systems (WellSky).

A valid driver's license and strong community outreach are preferred. Mon-Fri with flexibility as needed.

Qualifications

  • Graduate from a physical therapist curriculum approved educational institution.
  • Currently licensed in the state of Montana.
  • 1 year home health experience, preferred.
  • BLS/CPR required for this position.
  • Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements.
  • Knowledge of the local healthcare market and community outreach strategies.
  • Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR).
  • Clean driver’s license with a clean driving record.

Responsibilities

  • Understands and adheres to established policies and procedures.
  • Provides physician-prescribed physical therapy.
  • Improves or minimizes residual physical disabilities of the patient.
  • Returns the individual to optimum and productive level within the patient's capabilities.
  • Participates with all other home care personnel in patient care planning.
  • Directs and supervises personnel as required.
  • Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days.
  • Performs all skilled procedures as ordered by the physician.
  • Consult with physicians regarding changes in treatment.
  • Writes reports to physicians regarding patient's progress.
  • Instruct patients and family/significant others in-home programs and activities of daily living.
  • Participates in in-service programs and presents in-service programs as assigned.
  • Participates in Quality Assurance and Performance Improvement activities as assigned.
  • Attends all patient care conferences as scheduled.
  • Prepares clinical and progress notes.
  • Assists physician with evaluating level of function.
  • Helps develop the plan of care and revise it as necessary.
  • Consult with family and Agency personnel.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition under Agency defined time frames.
