Role Description

The Physical Therapist will provide contract, on-site care to patients in Northern Virginia, VA, delivering therapy services in the home setting. Day-to-day responsibilities include assessing patient functional status, developing individualized treatment plans, and implementing evidence-based therapeutic interventions to improve mobility, strength, balance, and endurance. The role involves regular home visits, monitoring progress, updating documentation, and collaborating with skilled nursing, occupational and speech therapy, and medical social work professionals. The Physical Therapist will educate patients and caregivers on exercises, safety, and equipment use, while promoting independence and optimal quality of life. This position also requires maintaining compliance with regulatory standards and Esprit’s clinical policies.

Qualifications

• Candidates should possess strong skills in Physical Therapy and Rehabilitation, including evaluation, treatment planning, and therapeutic interventions.
• Candidates should possess skills in Patient Care and Home Care, with a focus on safety, comfort, and person-centered service in the home environment.
• Candidates should possess foundational knowledge in Medicine relevant to physical therapy practice, including understanding of diagnoses, contraindications, and treatment precautions.
• Current state licensure as a Physical Therapist in Virginia and graduation from an accredited physical therapy program.
• Prior experience in home health or community-based rehabilitation is preferred.
• Strong written and verbal communication skills, including accurate clinical documentation and patient education.
• Ability to work independently, manage a caseload, and travel to patient homes throughout Northern Virginia.
