The Grand Rehabilitation & Nursing at Pawling is seeking a Physical Therapist (PT) for per diem coverage under new leadership. Join a facility investing in clinical programs, teamwork, and resident outcomes.

Per diem schedule offers flexibility while contributing to a supportive rehabilitation team in a modern program. Responsibilities include evaluating residents, designing individualized treatment plans, and guiding residents toward independence with evidence-based therapies.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State Physical Therapist License.
• Experience in Skilled Nursing Facility or Long-Term Care preferred.
• Strong communication and teamwork skills.
• Passion for helping residents achieve independence.

Responsibilities

• Evaluating residents and developing individualized treatment plans.
• Helping residents improve mobility, balance, strength, and overall function.
• Utilizing therapeutic exercises, manual techniques, stretching, and modalities to restore movement.
• Educating residents and families throughout the rehabilitation process.
• Monitoring progress and adjusting treatment plans as appropriate.
• Assisting residents with proper use of rehabilitation equipment.
• Supervising Physical Therapy Assistants when required.
• Collaborating with nursing, therapy, and interdisciplinary teams.
• Maintaining accurate documentation and compliance standards.
• Supporting a safe, clean, and resident-centered environment.

Your Role

As a Physical Therapist, you'll help residents regain strength, mobility, confidence, and independence while playing a key role in their rehabilitation journey.
