Job summary

Flagship Therapy in Upland, CA seeks a PRN Physical Therapist for weekend coverage within our in-house rehab program. You will screen and evaluate patients, develop individualized care plans, and deliver skilled therapy to geriatric patients in a post-acute setting.

Licensed PT required; new grads are welcome. Compensation ranges from $55 to $62 per hour based on experience, with benefits available and a supportive team culture focused on recovery and quality care.

Responsibilities

PHYSICAL THERAPIST - PRN - WEEKEND SCHEDULE

Upland Rehabilitation in Upland, CA is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Physical Therapist for weekend coverage for our in-house rehab program!

PRN: Weekend schedule with work-life balance

Pay: $55 - $62/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Physical Therapist license is required. Open to Physical Therapists at all experience levels. New Grads are welcome!
