Responsibilities

• Develops an overall therapy plan of care to include treatment and medical diagnosis of the warranted treatment.
• Plans duration and frequency of therapy to be provided.
• Completes all documentation required of observations, patient's response to treatment, treatment programs, and progress in the patient’s records.
• Completes all billing required on the date of the provided treatment.
• Communicates and coordinates patient care with other health team members.
• Supervises physical therapy assistants in accordance with regulations and standards of care.
• Helps residents improve mobility and pain relief using exercises, manual therapy, and devices.
• Communicates patient progress and projected milestones of recovery plan.

Education

Trenton Gardens Rehabilitation, a long term skilled nursing facility located in Mercer County, New Jersey is actively looking for a caring, dedicated per diem Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

PT Duties Include:

Develops an overall therapy plan of care to include treatment and medical diagnosis of the warranted treatment.

Plans duration and frequency of therapy to be provided.

• Utilizes standardized assessments upon the development of the plan of care to validate and monitor the need for skilled therapy services, and for the ongoing need for skilled therapy and the patient's progress from received.

• Completes all documentation required of observations, patient 's response to treatment, treatment programs, degree of progress and notable changes in patient's condition within required time in accordance with state and federal regulations and general standards of practice

• Completes all billing required on the date of the provided treatment

• Communicates and coordinates patient's care with other members of the patient' s health care

• Supervises physical therapy assistants in accordance with regulations and standards of care.

• Help Residents to improve their mobility and pain relief by using exercises, hands-on therapy, stretching, and physical therapy devices.

• Communicates with Residents progress and projected achievement of stages of their recovery plan.

QUALIFICATIONS:

• Graduate of an accredited Physical Therapy Program

• Current New Jersey State PT License

• Experience in skilled nursing/ long term care settings preferred
