Physical Therapist – Waverly, NY

We are looking to add skilled and caring Physical Therapists to the Waverly campus. Full‑time and per‑diem positions are available.

Per‑diem compensation: $55.00 / hour

Full‑time compensation: $33.02 – $49.95 / hour

Location

Elderwood at Waverly is located in Waverly, NY, between Elmira and Binghamton, on the Twin Tiers. The campus features a 200‑bed skilled nursing facility with secure memory care and Adult Medical Day Services, adjacent to an assisted living community. A spacious, up‑to‑date therapy gym accommodates residential subacute rehabilitation and outpatient physical, occupational and speech therapy. The location boasts a picturesque courtyard with a serene waterfall for all to enjoy. We are rated as a 4‑star facility on Care Compare.

Position Overview

Physical therapy staff members assist with ensuring the health and well‑being of residents by providing comprehensive, quality therapy services. State‑specific Physical Therapy license is required. We are looking to fill a full‑time position.

Responsibilities
  • Initiate skilled physical therapy treatments in accordance with physicians' orders; maintain required contact with physicians.
  • Maintain written evaluations, progress notes and discharge summaries for residents in the physical therapy program.
  • Confere with other health‑care staff regarding special needs of residents to enhance rehabilitation or maintenance of function.
  • Provide input for Resident Care Planning meetings and attend in the absence of the Director of Physical Therapy.
  • Maintain statistical records of residents' treatments.
  • Assist with in‑service training programs upon request.
  • Record and update residents' plan of care in the inter‑disciplinary Kardex.
  • Submit requisitions for supplies needed in the treatment of residents to the Director of Physical Therapy.
  • Assume all duties of the Director of Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
Qualifications
  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
  • Two years' experience in physical therapy preferred.
  • Interest in physical therapy treatment programs for geriatric and sub‑acute populations necessary.
  • Ability to direct others necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. The employee must be able to read, write, speak, and understand the English language at an intermediate or more advanced level to ensure a safe work environment and effective communication.
