Job summary

Sport & Spine Clinic of Greenwood is seeking a passionate Physical Therapist to join our outpatient team in Greenwood, WI. You will assess patients, develop and initiate personalized treatment plans, and provide direct patient care within established protocols.

Ideal candidates will have a CAPTE-accredited PT degree, Wisconsin license, CPR certification, and strong documentation skills. You will collaborate with physicians and insurance providers while supporting a friendly, collaborative work

Qualifications

• Licensed or eligible for Wisconsin Physical Therapy license
• CPR certification required
• Autonomy in outpatient setting preferred
• Proficient in EMR and MS Office

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies

Qualifications

• Graduate degree from CAPTE-accredited Physical Therapy
• Current or upcoming state of Wisconsin Physical Therapy license and CPR certification
• Ability to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans or willingness to learn
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office
