Job summary

Beachside Post Acute is seeking a licensed Physical Therapist to join our per diem team. You will evaluate residents, develop individualized plans, and provide skilled interventions to maximize mobility and independence. A CA license and at least 2 years of healthcare experience are required.

You'll collaborate with an interdisciplinary team, educate residents and families, and maintain thorough documentation while staying current with PT practice standards and regulations.

Qualifications

• Currently licensed as a Physical Therapist in California.
• Minimum of 2 years of healthcare experience, preferably in a skilled nursing facility.
• Knowledge of current physical therapy practice standards and regulations.

Responsibilities

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments.
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities.
• Monitor and document resident progress and adjust treatment plans as necessary.
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs.
• Maintain accurate and timely documentation of physical therapy services provided.
• Participate in team meetings, in-service education, and quality improvement initiatives.
• Maintain current knowledge of physical therapy practice and relevant regulations.
