Job summary

Brighten Life Home Health is seeking a Physical Therapist (PT) to join our part-time/PRN team in Utah. You will evaluate patients, develop individualized treatment plans, and deliver PT services in home health settings, focusing on improving mobility, function, and independence.

You will collaborate with patients, families, and the healthcare team to meet therapy goals and provide education for home exercise programs.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Utah.
• Strong clinical, assessment, and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a healthcare team.
• Strong organizational and documentation skills.

Responsibilities

• Evaluate patients and participate in therapy screenings.
• Develop and implement individualized treatment plans.
• Provide physical therapy services to improve mobility, strength, and function.
• Collaborate with patients, families, and the healthcare team to achieve therapy goals.
