Job summary

Hill Valley Healthcare is seeking a dedicated Physical Therapist to join our team on a full-time basis. You will assess referred patients and deliver direct rehabilitation care within a collaborative, inclusive environment.

Qualifications

• Must be a licensed Physical Therapist in the state of practice.
• Graduate from an accredited PT program with a valid degree.
• Ability to perform PT assessments and provide direct patient care.
• Experience with PCC EMR is a plus, CPR certification required.

Responsibilities

• Conduct physical therapy assessments of referred patients.
• Provide direct patient care and rehabilitation planning.
• Document sessions and progress in PCC EMR.
• Collaborate with care teams to support patient outcomes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
