Role Description

The Physical Therapist will provide in-person, house call therapy services to patients in the Stamford, CT area, focusing on assessment, treatment planning, and implementation of individualized rehabilitation programs. Day-to-day responsibilities include conducting comprehensive evaluations, developing evidence-based treatment plans, delivering hands-on interventions, and monitoring patient progress. The role involves close collaboration with other healthcare professionals, maintaining accurate documentation, and educating patients and caregivers on home exercise programs and safety strategies.
