We are seeking a licensed and compassionate Physical Therapist (PT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans.

Evaluate residents and develop individualized treatment plans based on clinical needs and physician orders.
Provide skilled physical therapy services in accordance with established care plans.
Monitor resident progress and modify treatment approaches as clinically appropriate.
Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
Collaborate with nursing, therapy, and interdisciplinary team members to achieve positive resident outcomes.
Participate in care planning, utilization review meetings, and interdisciplinary conferences.
Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and functional mobility.
Maintain established productivity, documentation, and quality standards.
Support regulatory compliance and survey readiness.
Promote resident safety, dignity, independence, and overall well-being.
Graduate of an accredited Physical Therapy program.
Current and active state license as a Physical Therapist.
Long-term care or skilled nursing experience preferred.
Strong clinical documentation and communication skills.
Excellent interpersonal and organizational skills.
Ability to work effectively within an interdisciplinary team.
Compassionate, patient-focused, and committed to delivering high-quality care.
Ability to stand, walk, bend, and assist residents throughout the workday.
Ability to safely lift, transfer, and support residents during therapy activities.
Ability to perform physical tasks associated with patient care and therapeutic treatment.
