Job summary

South Hadley Rehab and Nursing Center in South Hadley, MA is seeking a Full-time Physical Therapist (PT) to join our in-house therapy team. The PT will perform initial evaluations, develop personalized treatment plans, and deliver services to restore mobility and reduce pain.

You will collaborate with nurses, OTs, SLPs and physicians; educate patients and families on home programs; supervise PTAs; and maintain timely clinical documentation in accordance with state and federal regulations.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
• Current State Physical Therapist license in good standing (or eligible).
• Current CPR certification (or willingness to obtain upon hire).
• Strong clinical, organizational, and interpersonal skills.
• Previous experience in a skilled nursing or long-term care setting is preferred but not required.

Responsibilities

• Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.

• Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
• Participate in care planning, team meetings, and quality improvement initiatives as needed.
