Job summary

Your Health Organization is seeking a skilled Physical Therapist to serve patients in the Aiken and North Augusta area. The role includes home visits, care facility visits, clinics, and Telehealth, with a salary-based structure and monthly bonus opportunity.

The ideal candidate is compassionate, communicates effectively, and can coordinate with the health team while educating patients. Full-time, 8-hour days, Monday to Friday, with travel between offices and facilities expected.

Qualifications

• Successful completion of a Physical Therapy program and licensed by the SC state board.
• Current CPR certification is required.
• Previous healthcare experience is preferred.

Responsibilities

• Perform home, ALF/ILF visits, clinics, and Telehealth sessions.
• Assess patient health and psychosocial status; document findings.
• Educate patients and families on health maintenance and treatment plans.
• Coordinate referrals with the health care team and community resources.
• Maintain confidentiality and comply with treatment authorization requirements.

We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Aiken and North Augusta area. Physical Therapist role performs visits in homes, ALF's, and ILF's, clinics, and via Telehealth. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

What Are We Looking For?

Your Health is currently looking for a Physical Therapist to join our growing primary care family. You must be a compassionate and empathetic clinical professional with the ability to provide the highest standards of care to our patients. It is also important to have excellent communication and interpersonal skills and the ability to interact in a positive and motivating manner. As a Physical Therapist here at Your Health, you will obtain complete health and psychosocial history and records findings in a systematic format. Perform complete physical assessment, while providing health education and counseling. Arrange referrals to other members of the health team as needed and to community resources. Physical Therapist must use appropriate reporting mechanisms as required by law, respect the dignity, confidentiality, and privacy of patients, obtain appropriate authorization and consents for treatments, review patient’s medical diagnosis, prognosis, procedures, and clinical course. And market and provide community education.

Qualifications

• Successful completion of a Physical Therapy training program and certified thought the therapy state board
• A current license to practice in the state of SC.
• Current CPR Certificate.
• Previous experience in healthcare setting is preferred.
• Must be able to work flexible hours and travel between offices, facilities, and other places.
• Ability to utilize electronic documentation required'
• At least one year experience in the specialty field you are applying for
• Experience and/or interest in Pelvic Health Preferred
