Premier Physical Therapy and Sports Performance in Smyrna, DE is seeking a full-time Physical Therapist with an entrepreneurial spirit for our outpatient clinic. We emphasize evidence-based care and professional growth, offering a supportive culture focused on excellent patient outcomes.

You will assess patients, develop treatment plans, document progress, and collaborate with physicians and insurers. Applicants must graduate from an accredited PT program and hold a Delaware PT license and CPR

Qualifications

• Graduate from an accredited Physical Therapy Program.
• Current State of Delaware Physical Therapist (PT) license, CPR certification.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician’s referral and patient records to determine PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Supervise assistants and rehabilitation technicians.
• Develop a written plan of care and comprehensive program to attain goals.
• Implement treatment program and essential PT procedures.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes and reports.

What You'll Do:

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate from an accredited Physical Therapy Program.
• Current State of Delaware Physical Therapist (PT) license, CPR certification.
• Outgoing and energetic personality.
