Job summary

Daley And Associates, LLC is seeking a Physical Therapist to join a hospital-based team in Sierra Vista, Arizona. The PT will evaluate patients, develop plans of care, and provide evidence-based interventions to improve mobility, strength, balance, and functional independence within an interdisciplinary setting.

The role requires an accredited PT program graduate with an active Arizona PT license and BLS certification, plus experience in rehab or acute-care settings.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapy license in the state of practice (Arizona).
• BLS certification is required.

Responsibilities

• Evaluate patients and develop individualized physical therapy plans of care based on clinical and functional needs.
• Provide evidence-based interventions focused on improving mobility, strength, balance, pain management, and functional independence.
• Monitor patient progress and adjust treatment plans and goals as appropriate.
• Document evaluations, treatments, progress, and recommendations in accordance with clinical and professional standards.
• Collaborate with physicians, nursing staff, occupational and speech therapists, case management, and other members of the interdisciplinary care team.
• Educate patients and caregivers on mobility, safety strategies, therapeutic exercises, and continued care needs.
• Participate in discharge planning and make recommendations for continued therapy, equipment, or supportive services as appropriate.
