Job summary

Amedisys is seeking a Physical Therapist to support patients in the Seattle-area home health care setting. You will assess and treat patients in their homes, develop individualized plans, and collaborate with caregivers and the care team to promote healing at home.

Qualifications

• Current license to practice physical therapy in the state you work in.
• Graduate from CAPTE-accredited physical therapy program.
• At least 1 year of home health PT experience preferred.
• If less than 1 year of experience, regional clinical mentoring may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days at minimum, modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
