Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our Schererville team. You will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and maintain independence for older adults.

In this role you will provide skilled PT interventions, supervise assistants as needed, document in EMR, and collaborate with an interdisciplinary team to deliver patient-centered care in assisted living and independent living settings.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
