Job summary

San Bruno Skilled Nursing is seeking a compassionate Physical Therapist to join our full-time, in-house rehabilitation team in San Bruno, CA. You will evaluate residents and develop individualized treatment plans to improve mobility and independence.

New graduates are welcome. The position offers mentorship, collaboration with a multidisciplinary team, and opportunities to advance clinical skills while focusing on resident outcomes.

Qualifications

• Active California Physical Therapist license.
• Strong clinical assessment and documentation skills.

Responsibilities

• Evaluate residents and develop PT treatment plans.
• Provide skilled PT services to improve mobility, strength, balance, coordination, and functional independence.
• Monitor resident progress and adjust treatment plans as appropriate.
• Document evaluations, treatment sessions, progress, and discharge recommendations timely.
• Collaborate with nursing, OT, ST, physicians, and the interdisciplinary team.
• Educate residents, families, and caregivers on therapy programs and home exercises.
• Supervise PTAs, as applicable.
• Promote resident safety, dignity, independence, and quality of life.
