Qualifications

• Licensed PT in the state or eligible for licensure

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans with long-term rehabilitation goals
• Deliver hands-on therapy interventions to support recovery and enhance quality of life for older adults
• Help patients restore functional mobility and safely perform daily activities with confidence
• Partner with interdisciplinary teams, caregivers and nursing staff to drive outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed
