Willow Tree is seeking a licensed Physical Therapist to provide rehabilitative therapy to residents in accordance with regulatory and clinical standards. You will screen and evaluate patients with cognitive and ADL challenges, develop and document care plans, and support recovery and functional improvement.
You will partner with the Director of Rehabilitation Services, supervise PT assistants, and contribute to discharge planning and outcome evaluation.

The Physical Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.
This position reports to the Director of Rehabilitation Services.
