Job summary

AdviniaCare at Vinnin Square is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for our residents.

As a licensed PT, you will collaborate with staff to assess needs, monitor progress, and educate residents and families on therapeutic exercises and techniques. Commitment to patient-centered care is essential.

Qualifications

• Valid state license as a Physical Therapist.
• Experience working in a rehab or interdisciplinary team is preferred.
• Ability to design and implement individualized treatment plans.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.
• Educate residents and families on therapeutic exercises and techniques.
