Job summary

Family PT in California seeks dedicated Physical Therapists to join outpatient clinics in Roseville and Rocklin. You will work alongside experienced clinicians, treating a diverse caseload while helping patients improve function across orthopedic, geriatric, and neurological conditions.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Active California Physical Therapist license or eligibility to obtain licensure.
• Familiarity with anatomy, physiology and PT principles.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct comprehensive 60-minute initial evaluations to assess patient needs and goals.
• Deliver evidence-based, patient-centered treatment plans using manual therapy and therapeutic exercise.
• Provide individualized care during 30-minute follow-up visits and adjust plans as needed.
• Manage discharge planning and home exercise program transitions.
• Maintain accurate documentation of evaluations, treatments, and progress in the EHR/EMR.
• Coordinate with physicians, PTAs, aides, and other providers to optimize outcomes.
• Build relationships with local referring physicians to support continuity of care.

Responsibilities

• Conduct comprehensive 60-minute initial evaluations to assess patient needs, establish goals, and develop individualized treatment plans.
• Deliver evidence-based, patient-centered treatment plans using manual therapy, therapeutic exercise, and other appropriate interventions.
• Provide individualized care during 30-minute follow-up visits, monitoring progress and adjusting treatment plans as needed.
• Manage discharge planning and help patients transition smoothly to home exercise programs or continued care.
• Maintain accurate, timely documentation of evaluations, treatments, and patient progress in the EHR system.
• Collaborate with physicians, PTAs, Aides, and other healthcare providers to coordinate care and optimize patient outcomes.
• Build and maintain relationships with local referring physicians to support continuity of care and strong community partnerships.

Requirements

• Doctor of Physical Therapy (DPT) degree from an accredited program
• Active California Physical Therapist license or eligibility to obtain licensure
• Strong foundation in anatomy, physiology, and physical therapy principles
• Familiarity with EHR/EMR systems for clinical documentation and treatment planning
• Strong communication and interpersonal skills, with the ability to collaborate effectively with patients and healthcare providers
• Experience treating orthopedic, geriatric, or neurological populations is a plus
