Job summary

AdviniaCare Summit Commons is seeking a compassionate Physical Therapist to join our interdisciplinary team. You will design and implement personalized treatment plans to help residents regain independence and improve function at every level.

You will collaborate with rehabilitation staff, educate residents and families on exercises, monitor progress, and maintain precise records. A Rhode Island license is required; a $5,000 sign-on bonus is offered for full-time employment.

Qualifications

• Licensed Physical Therapist license is required.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
