About You (Qualifications)

• You are a Licensed Physical Therapist and can work with a diverse patient population at all experience levels.
• You’re passionate about making a positive impact on patients’ lives and helping them regain independence.
• You’re committed to delivering evidence‑based treatments and collaborating with an interdisciplinary team.

Responsibilities

• Assess, evaluate, and develop treatment plans for patients with cognitive and/or ADL physical functioning disorders.
• Administer rehabilitation interventions including ADL retraining, therapeutic exercises, modalities, and the use of assistive devices.
• Regularly assess patient progress, adjusting treatments accordingly, and communicate updates with the healthcare team.
• Provide counseling and consultation to patients, families, caregivers, and service providers.
• Document treatment plans and outcomes in line with regulatory and clinical practice standards.
• Contribute to patient‑centered care by evaluating treatment effectiveness and making necessary referrals.
