Flagship Therapy's Granite Mesa Health Center in Marble Falls, TX, is seeking a full-time Physical Therapist to join our in-house rehab program within a skilled nursing setting.

Applicants must hold an active Physical Therapist license and may be new grads. You will conduct screenings, plan and deliver rehabilitative treatments, and contribute to discharge summaries while adhering to regulatory standards. We offer a supportive team and comprehensive benefits.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan (manual techniques, ambulation, exercises, modalities, devices, wound care).
• Provide counseling to patients, families, and other providers about physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.

Granite Mesa Health Center in Marble Falls, TX, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Physical Therapist license is required.
