Job summary

Brookestone Home Health is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a Part-Time or PRN basis, delivering one-on-one care to patients in their homes across the Omaha metro area.

You will evaluate and treat patients, develop individualized plans, and collaborate with the care team to improve mobility and achieve independence, while maintaining compliance with policies and standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Nebraska Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Ability to travel throughout the Omaha service area.

Responsibilities

• Conduct physical therapy evaluations and reassessments.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise, gait training, balance training, transfer training, and fall prevention education.
• Assess the need for assistive devices and durable medical equipment.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Monitor and document patient progress toward established goals.
• Collaborate with physicians, nurses, and other healthcare professionals regarding patient care.
• Complete clinical documentation accurately and in a timely manner.
• Maintain compliance with agency policies, professional standards, and regulatory requirements.

Brookestone Home Health is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a Part-Time or PRN basis. Position Summary

The Physical Therapist is responsible for evaluating and treating patients in their homes, developing individualized treatment plans, and collaborating with the interdisciplinary care team to achieve optimal patient outcomes.
