Job summary

Caring Hand Home Health Solutions, Inc. in Oakland, CA is seeking a licensed Physical Therapist to provide skilled services in clients' homes on a per diem basis. The role offers a per-visit rate of $170–$190 and requires a CA PT license and CPR certification, with a valid driver’s license preferred.

You will evaluate patients, develop treatment plans, educate families, supervise aides, and handle documentation while coordinating with physicians for patient progress.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
