Job summary

United Energy Workers Healthcare seeks a skilled Physical Therapist to deliver individualized rehabilitation in a home health setting, helping patients regain independence and mobility while reducing pain.

You will assess patients, develop treatment plans, educate families, document progress, and collaborate with physicians and nurses to ensure comprehensive care and successful discharge planning.

Qualifications

• Licensed Physical Therapist with a current state license.
• Geriatric care experience is preferred.
• Excellent assessment, communication and organizational skills.
• Ability to work independently with flexibility.
• Experience with EMR and MS Office.

Responsibilities

• Conduct patient assessments and create personalized treatment plans.
• Provide hands-on interventions to improve mobility, strength and flexibility.
• Educate patients and families on exercises and home programs.
• Maintain accurate progress notes and treatment documentation.
• Collaborate with physicians, nurses, and other healthcare professionals.

We are seeking a skilled and empathetic Physical Therapist to join our team. In this role, you will be responsible for providing individualized rehabilitative care to our patients in a home health setting. Your expertise will help patients regain independence, reduce pain, and improve mobility, ultimately enhancing their quality of life.

Responsibilities

• Patient Assessments: Conduct thorough evaluations of patients’ physical abilities, medical history, and limitations to create personalized treatment plans


• Treatment Plans: Develop individualized rehab programs based on assessments and patients’ goals, including therapeutic exercises, manual techniques, gait training, and modalities


• Patient Care: Provide hands‑on interventions to enhance mobility, strength, and flexibility, while monitoring progress and adjusting treatment as necessary


• Family & Caregiver Support: Educate patients and their families on exercises, techniques, and injury prevention strategies to continue rehabilitation outside of sessions


• Documentation: Maintain accurate and up‑to‑date patient records, including treatment plans, progress notes, and outcomes


• Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care and recovery


• Discharge & Follow‑Up: Discharge patients when goals are met, providing recommendations for continued care or referrals as needed


• Other Duties: Perform other related tasks as assigned, ensuring flexibility in patient care as necessary

Qualifications

• Qualifications: Licensed Physical Therapist with a valid state license


• Experience: Previous experience in a clinical or home health setting, especially with geriatric care, is preferred


• Skills: Strong assessment, communication, and organizational skills


• Patient‑Centered Care: Empathy and the ability to work with patients facing various physical challenges


• Independence: Ability to work independently with a flexible schedule


• Tech Proficiency: Experience with electronic medical records (EMR) and Microsoft Office Suite


• Other: Must pass a criminal background check and drug screen
