Job summary

Blue Stone Therapy is seeking a Physical Therapist to provide rehabilitation services in a skilled nursing/post-acute setting for adult and geriatric patients at Accura HealthCare of Pierce in Pierce, NE. You will evaluate, plan, and implement individualized treatment programs to improve gait, balance, mobility, and functional independence.

Responsibilities include patient assessments, documentation, discharge planning, and interdisciplinary collaboration.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.

• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at [Accura HealthCare of Pierce] in [Pierce, NE].

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.
