Orleans Essex VNA & Hospice in Vermont is seeking a dedicated Physical Therapist to join our home health team serving Orleans and northern Essex counties. You will provide personalized, evidence-based care to patients in their homes, helping improve mobility, reduce pain, and enhance quality of life.

This role offers autonomy, variety across pediatric and geriatric patients, and opportunities to work with interdisciplinary teams and EMR systems.

Qualifications

• Active Vermont PT license required.
• Experience in home health, outpatient, or inpatient settings.
• Strong knowledge of anatomy, physiology, and rehabilitation techniques.
• Experience with EMR documentation systems.
• Excellent communication and patient education skills.
• Ability to work independently and manage time effectively.
• Reliable transportation to travel within service area.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide hands-on therapy including orthopedic rehab and trigger point therapy.
• Deliver care to patients across various age groups.
• Coordinate with healthcare teams on care plans and discharge planning.
• Document patient progress using EMR systems.
• Educate patients and families on exercises, recovery plans, and injury prevention.
• Adjust treatment plans based on patient progress and needs.

Pay

Pay: $34.75 - $56.74 per hour
