Job summary

Pomera Care LLC is seeking a licensed Physical Therapist to provide high-quality rehabilitative care to diverse patient populations in a multi-specialty clinic. You will create personalized care plans to promote recovery, mobility, and overall well-being with an energetic, patient-centered approach.

You will work with adults and geriatric patients, using manual therapy and education to improve function, reduce pain, and support long-term wellness.

Qualifications

• Licensed Physical Therapist with clinical practice authorization.
• 1+ years of experience in outpatient, inpatient, or rehabilitation settings.
• Fluency in Spanish to support patient communication and education.

Responsibilities

• Develop and implement individualized treatment plans for adult and geriatric patients.
• Deliver manual therapy interventions including trigger point techniques, soft-tissue mobilization, and joint mobilization.
• Monitor patient progress, ensure safety during all activities, and maintain patient motivation throughout rehabilitation.
• Educate patients and families on therapeutic exercises, home recovery strategies, and long-term wellness practices.
• Document evaluations, treatment sessions, and progress notes in the electronic health record per regulatory standards.
• Collaborate with interdisciplinary teams to support coordinated, patient-centered care.

Qualifications
