Job summary

New Franklin Center for Rehabilitation and Nursing in New York seeks qualified physical therapists to join our rehabilitation team in full-time, part-time, or per diem capacities.

The Physical Therapist assesses referred patients, develops treatment plans, documents progress, and educates patients and families while supervising PTA staff and coordinating with the Rehab Director.

Qualifications

• Licensed Physical Therapist in NY or actively pursuing licensure.
• Ability to assess and plan rehabilitation for referred patients.
• Strong documentation and patient/family education skills.

Job description

New Franklin Center for Rehabilitation and Nursing is seeking qualified Full Time/ Part Time and Per Diem Physical Therapists to join our dynamic rehabilitation team.

The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Applicants must have ccurrent state license as a Physical Therapist or proactively in the process of the application process for current state licensure as a Physical Therapist.

Duties Include:

• Implement and document patient status under the direction of the Rehab Director
• Screening patients
• Performing evaluations and developing initial and interim treatment, care and discharge plans
• Documenting progress toward identified objectives
• Overseeing Physical Therapy Assistant(s)
• Developing and implementing new program/techniques to enhance physical therapy services.
