The KD model of care includes clinicians from across multiple disciplines providing exemplary care in areas including primary care medicine, psychiatry, dentistry, therapeutic services, and behavioral health. Responsibilities
The Physical Therapist will be responsible to provide physical therapy services to our patients with Intellectual Disabilities
Clinical
1. Plan and administer medically prescribed physical therapy.
2. Administer physical therapy treatments by utilization of soft tissue manipulation, mobilization, massage, ROM, therapeutic exercises, therapeutic activities and gait training.
3. Direct treatments given by aides, technicians, and assistants.
9. Evaluate results of physical therapy by observing, noting, and evaluating patient's progress.
10. Evaluate effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress, and recommending adjustments and modifications if indicated.
11. Interview patients, care givers, guardians as needed to make recommendations.
12. Assure continuation of therapeutic plan by designing and instructing patients, families, and caregivers in home exercise programs, recommending and / or providing assistive equipment, and recommending outpatient or home health follow-up programs.
13. Document patient care services by charting in electronic health records (EHR).
14. Adhere to the highest standards of professional ethics at all times.
15. Model an attitude of mentoring and teamwork.
16. Work with Leadership Team in development of, implementation of and adherence to best-practice standards and clinical protocols.
Education / Training / Licensure
Graduate degree, Physical Therapy
Professional licensure
BLS certification
Preferred Experience
Minimum 1 year experience working with patients with IDD.
