• Demonstrated skills in patient care, including assessment, monitoring, and ongoing evaluation of functional status.
• Experience or aptitude in home care or community-based care models, supporting residents in their living environment.
• Current licensure or eligibility for licensure as a Physical Therapist in the state of practice.
• Experience working with older adults or in senior living settings preferred.
• Effective communication and teamwork skills, with the ability to collaborate with interdisciplinary staff and families.
• Ability to work on-site in the Greater Myrtle Beach Area, manage a clinical caseload, and maintain accurate documentation in accordance with regulatory and organizational standards.

Role Description
