Job summary

Cherokee Nation Home Health Services in Oklahoma seeks a licensed Physical Therapist to provide in-home care, evaluate mobility, and create individualized treatment plans that support safe recovery.

You will work with physicians, nurses, therapists, and caregivers to empower patients, educate families, document progress, and adapt plans as needed in a home-health setting.

Qualifications

• Current Oklahoma license as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Valid driver;s license, reliable transportation, and automobile insurance.
• Home health experience preferred but not required.

Responsibilities

• Evaluate patients physical abilities and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Assess home environments and recommend equipment or modifications to improve safety and independence.
• Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies.
• Document patient progress and maintain accurate clinical records in compliance with agency and regulatory standards.
• Collaborate with interdisciplinary team members to coordinate care and achieve optimal patient outcomes.
• Monitor patient responses to treatment and modify care plans as needed.

In the role of the Home Health Physical Therapist (PT), you will provide therapy services to patients in their homes, helping them regain mobility, improve functional independence, manage pain, and enhance their overall quality of life.
