Elara Caring is seeking a Physical Therapist to deliver high-quality care in patients' homes, advancing our home health leadership. You will evaluate patients, design individualized treatment plans, and coordinate with physicians and caregivers to ensure progress and safety.
This role requires an APTA-approved program graduate with an active PT license, at least one year in healthcare, and reliable transportation for travel between patient homes.

As a Physical Therapist, you’ll contribute to our success in the following ways: Evaluate patients and develop individualized treatment plans. Provide physical therapy services in the home setting. Monitor patient progress and adjust care plans as needed. Collaborate with physicians, caregivers, and the interdisciplinary care team. Complete timely and accurate clinical documentation. Supervise therapy assistants and support staff, as appropriate.
