Job summary

Elara Caring is seeking an experienced Physical Therapist to join our home health team. You will evaluate patients, develop customized treatment plans, and deliver therapy services in patients' homes, ensuring timely progress updates and care plan adjustments.

Collaborate with physicians and caregivers, maintain thorough clinical documentation, and supervise therapy assistants as needed. A valid PT license, state eligibility, and at least 1 year of healthcare facility experience are required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver’s license, or reliable transportation to travel between patient homes and assigned locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
